Foam, gel, air or hybrid
| Material | Strengths | Trade-offs |
|---|---|---|
| Foam | Comfortable, lightweight, mild pressure spreading | Basic flat foam has limited shaping |
| Gel and foam | Pressure relieving materials used in higher grades | Heavier than foam or air |
| Air cells | Lightweight, strong pressure relief | Can be a little less stable; needs inflation checks |
This summary follows the Spinal Injuries Association (SIA) cushion sheet. The SIA also grades cushions from low to very high pressure care. Basic flat foam suits people who can shift their weight and do not sit for long. Higher grades suit people with less skin sensation, people who sit for long periods, or people with a history of skin problems. If you are in that group, see cushions for pressure sores and ask your wheelchair service for an assessment.
Getting the size right
- Measure the seat of the wheelchair from side to side and front to back.
- Pick a cushion with the same width and depth. Drive Medical, for example, sells its molded foam cushion in 16 by 16, 18 by 16, 20 by 16, 18 by 18 and 20 by 18 inch sizes, and its gel and foam cushion up to 22 by 18 inches.
- Check thickness. Drive lists its foam cushion at 2 inches in one place and 2.5 inches in its spec table, and its gel and foam cushion at 3 inches. A thicker cushion sits you higher, so recheck your foot position and your reach to the wheels afterward.
- Check the weight rating; both Drive examples list 250 lb.
MedlinePlus says to make sure your wheelchair itself is the right size and to have a provider or physical therapist check the fit once or twice a year, especially if you gain weight or feel pressure anywhere.
Covers and setup
- Cover. SIA recommends a breathable, moisture-wicking cover. Drive's covers are listed as low-shear, vapor permeable and water-resistant, with a non-slip vinyl base.
- Ties. Drive's foam cushion includes 12 inch tie straps so it stays put.
- Orientation. Shaped cushions have a front and back. Set it the way the maker shows.
- Hand check. Slide your hand under the lowest bony point of your pelvis while seated. SIA says you should feel about 1/2 to 1 inch of space. Permobil gives the same target for its ROHO air cushions.
Care and when to replace
SIA advises checking your cushion's setup daily and cleaning it weekly. Wipe foam cores with a damp cloth rather than soaking them; wash covers and dry them fully. Replace a cushion that is worn or damaged. Warning signs SIA lists include foam that stays squashed or lumpy, gel that hardens or leaks, air cushions that keep losing air, and any loss of firmness or even support.
Cushion habits that protect skin
- Shift your weight every 15 to 20 minutes, leaning forward and to each side, as MedlinePlus advises.
- Do not use a donut-shaped cushion, which MedlinePlus says to avoid.
- Check your skin daily, or have a caregiver check it.
- For back support, see wheelchair back cushions.
What to look for when you buy
Molded foam wheelchair cushion
Best for: Lower-risk users who move often and want comfort
Drive Medical lists its molded foam cushion as designed for low-risk users, with a low-shear water-resistant cover, a non-slip base and tie straps.
Watch out: SIA notes basic foam has limited shaping; not enough for people at high risk of pressure injury.
See options on AmazonGel and foam pressure redistribution cushion, 3 inch
Best for: People who sit for long periods
Drive Medical describes its 3 inch gel and foam cushion as pressure redistributing, with a gel bladder inside a foam shell; SIA lists gel and foam mixes among its higher pressure care grades.
Watch out: Heavier than foam or air, per SIA.
See options on AmazonAdjustable air cell cushion
Best for: People needing more pressure relief, ideally after a seating assessment
SIA lists air-based cushions in its very high pressure care grade, and Permobil describes ROHO air cells that are inflated through valves and tuned to the user.
Watch out: SIA says air cushions can be a little less stable, and Permobil warns that under- or over-inflation reduces the benefit.
See options on AmazonSpare breathable cushion cover
Best for: Keeping a cover on while one is washed
SIA says to always use a suitable cover and to dry washed covers fully before reuse.
Watch out: Buy the exact size and model the cover is made for.

- What is the best type of wheelchair cushion?
- It depends on your risk of pressure injury. SIA grades basic foam for lower risk and gel and foam mixes or air for higher risk; a wheelchair service can assess you.
- What size wheelchair cushion do I need?
- One that matches your seat width and depth, such as 18 by 16 inches for an 18 inch wide, 16 inch deep seat.
- Are gel or foam wheelchair cushions better?
- Foam is lighter; gel and foam mixes add comfort and support but weigh more, according to SIA.
- Can I use a donut cushion in a wheelchair?
- No. MedlinePlus says not to sit on donut-shaped cushions.
- How often should I replace a wheelchair cushion?
- When it is worn or damaged, such as foam that stays flat, gel that hardens or leaks, or air that keeps escaping.
