Unloader knee brace: how it shifts load, and whether your knee is a match

Quick answerAn unloader knee brace is built for osteoarthritis that is worse on one side of the knee. AAOS OrthoInfo explains that it shifts weight away from the affected part of the joint, while a support brace carries load across the whole knee. It suits people whose arthritis is mainly on the inner or outer side, and it has to be ordered for the correct side and leg. If your whole knee is worn, or you are not sure which side is affected, ask your doctor before buying one.

How an unloader brace works

The knee has two main weight-bearing compartments, the inner (medial) side and the outer (lateral) side. Arthritis often wears one more than the other. An unloader brace uses a rigid frame, a hinge, and straps that apply gentle pressure to nudge the knee's alignment so the worn side carries less of your weight. OrthoInfo says a brace may be especially helpful if the arthritis is centered on one side of the knee, and describes the unloader as shifting weight away from the affected portion.

Cleveland Clinic calls unloader braces the most common type of brace providers suggest for knee arthritis, and says they move some weight off the knee to other parts of the leg. DonJoy describes its ROAM OA unloader as being for unicompartmental osteoarthritis, the one-sided kind.

Medial or lateral, left or right: getting the side right

This is the most common ordering mistake. An unloader pushes in one direction, so the version for a worn inner side of the left knee is a different setup from one for the outer side. DonJoy, for example, sells off-the-shelf ROAM OA versions in two fitment options: left medial/right lateral, or right medial/left lateral. Before ordering, write down which knee and which side your clinician says is worn, and match it to the maker's chart.

Sizing, and custom versus off the shelf

Unloaders are sized more carefully than sleeves because the frame must sit close to the leg. DonJoy sizes its off-the-shelf ROAM OA by three measurements: thigh, knee center and calf. Its smallest size, for example, covers a thigh of 13.0 to 18.5 inches, a knee center of 12.0 to 14.0 inches, and a calf of 10.0 to 14.0 inches. Measure all three, and if they point to different sizes, ask the maker or a fitter.

Custom unloaders are measured by a clinician or orthotist with a dedicated tool. DonJoy notes that those custom measurements are not the same as off-the-shelf measurements, so do not reuse one for the other. A custom brace can make sense for unusual leg shapes or if off-the-shelf models slide. DonJoy lists the ROAM OA for conservative care or before surgery.

Wearing it well

What to look for when you buy

Off-the-shelf medial unloader knee brace

Best for: One-sided osteoarthritis on the inner side of the knee

OrthoInfo says an unloader shifts weight away from the affected part of the knee. This version is set up to push load away from the inner side; confirm with your clinician that this is the worn side.

Watch out: Ordering the wrong side or leg makes it useless. Check the fitment chart twice.

Lateral unloader knee brace

Best for: One-sided arthritis on the outer side of the knee

The same principle in the other direction. DonJoy's off-the-shelf range pairs left medial with right lateral and vice versa, which shows how side-specific these braces are.

Watch out: Less common, so fewer size options. Confirm the side on your imaging report.

Low-profile unloader with dial tightening

Best for: People who want to fine-tune pressure during the day

DonJoy's ROAM OA uses hinges, adjustable straps and a Boa dial. A dial lets you adjust tension in small steps without redoing every strap.

Watch out: Still a rigid brace. Sizing by thigh, knee and calf has to be right.

Full-knee support brace (non-unloading)

Best for: Arthritis spread across the whole joint

OrthoInfo describes a support brace as helping carry the entire knee load, which suits wear that is not concentrated on one side.

Watch out: Will not offload one compartment. Not the right tool for clearly one-sided arthritis.

When to talk to a doctor: Confirm with a doctor which side of your knee is affected before buying an unloader, since the wrong setup can add strain. Check with a clinician first if you have poor circulation, diabetes with reduced feeling in your legs, or fragile skin, and stop wearing it if you notice numbness, tingling, or sores.

Questions people ask

Do unloader knee braces work?
OrthoInfo says a brace can help with stability and function and may be especially helpful when arthritis is centered on one side of the knee. Results vary, and a brace works best alongside exercise.
Who should not use an unloader brace?
People whose arthritis is spread across the whole knee, whose pain is mainly behind the kneecap, or who have not been diagnosed. Ask your doctor first if you have circulation problems or fragile skin.
How do I know if I need a medial or lateral unloader?
Your X-ray or clinician tells you which side of the knee is worn. Most makers sell side-specific versions, so match the side and leg exactly.
Should an unloader brace be custom?
Off-the-shelf models work for many people. A custom brace, measured by a clinician, may suit unusual leg shapes or braces that keep sliding.
Can you walk all day in an unloader brace?
Some people wear one for long periods, but build up gradually and check your skin. Cleveland Clinic notes wear time depends on why you need the brace.
