What L-theanine is
MSK's About Herbs database describes L-theanine as a water-soluble amino acid found in green tea and mushrooms. It crosses into the brain, where it is thought to have calming effects through inhibitory neurotransmitters and changes in serotonin and dopamine, and may affect alpha brain waves linked to attention.
Purified L-theanine is sold as a supplement for its perceived relaxing and antioxidant effects.
What the research shows
| Use | Evidence summary |
|---|---|
| Focus with caffeine | Small to moderate short-term gains in some attention tests |
| Focus alone | Faster choice reaction time in one analysis; not confirmed by all tests |
| Sleep | Small studies suggest improvement |
| Anxiety | Mixed in small studies; reduced symptoms in some anxiety disorder trials |
Cognition and mood. A 2025 meta-analysis in Nutrition Reviews included 50 randomized trials in healthy people, 15 of which fed into at least one analysis. Theanine plus caffeine showed small to moderate benefits in the first and second hour on tests such as attention switching and vigilance accuracy, and theanine alone improved choice reaction time. The authors noted wide uncertainty.
Cognition, stricter look. A 2025 meta-analysis in the Journal of Clinical Medicine, with 5 trials and 148 healthy adults, found a dose-dependent effect on some visual processing tests but no significant effect on simple reaction time or the Stroop test. It called the evidence promising but not completely conclusive.
Mental health conditions. A 2024 systematic review of 11 trials found L-theanine, often added to other treatments, reduced symptoms more than control in schizophrenia, anxiety disorders and ADHD, but said more studies are needed.
L-theanine and caffeine
Many focus products pair L-theanine with caffeine. MSK notes that in human studies L-theanine changed some effects of caffeine on brain blood flow, cognition and mood, and other studies suggest the two can counteract each other's individual effects. The Nutrition Reviews meta-analysis found the combination performed better than placebo on several short-term tests.
MSK lists green tea as the food source of L-theanine.
Other research areas
- Mental health trials. The 2024 review's 11 trials came from Israel, Iran, the USA, Japan, Australia and Italy and covered schizophrenia, ADHD, OCD, major depression, sleep disorders, generalized anxiety and Tourette syndrome.
- Stroke. MSK notes population data link green tea drinking to stroke prevention, but it is unclear whether L-theanine alone has that effect.
- Cancer care. Preliminary data suggest theanine with cystine may reduce oxaliplatin nerve damage and esophagitis during chemoradiotherapy. These uses belong with your oncology team.
- Lab studies. Animal and cell studies suggest brain-protective effects, but MSK cautions that L-theanine may also alter neurotransmitter levels.
Safety
- Limited human data. MSK says most purported effects are not based on clinical trials.
- Sedatives. In animal models, L-theanine had additive effects with midazolam; human relevance is unknown. Ask your doctor if you take sedatives or sleep medicines.
- Chemotherapy. MSK advises people on chemotherapy to discuss L-theanine with their physician, and notes that EGCG in green tea reduces the activity of the drug bortezomib.
- Pregnancy and medicines. NOW Foods advises consulting a physician if pregnant, nursing, taking medication or with a medical condition.
- Mental health conditions. Use only as an add-on with your care team, not instead of treatment.
