What bacopa is
Bacopa monnieri is an herb used for many centuries in Ayurveda, India's traditional medicine, to improve memory, according to a 2022 review in the Interactive Journal of Medical Research. It is also known as brahmi or water hyssop.
A 2024 systematic review in Antioxidants lists active compounds including bacosides A and B, betulinic acid, loliolide, asiatic acid and quercetin. Lab research suggests antioxidant and nerve-protective actions. Supplements are often standardized to a percentage of bacosides.
What the research shows
| Use | Evidence |
|---|---|
| Age-associated memory impairment | Improved memory tests in a 12-week RCT |
| Alzheimer's disease | No difference from placebo or donepezil; very low certainty |
| Broader brain and mood outcomes | 22 clinical trials report benefits; quality varies |
Memory in older adults. In a double-blind trial published in the Indian Journal of Psychiatry, people with age-associated memory impairment, without dementia, took 125 mg of standardized bacopa extract or placebo twice daily for 12 weeks. The bacopa group improved on mental control, logical memory and paired associate learning.
Alzheimer's disease. The 2022 review found 5 eligible trials. Three combined bacopa with other herbs, doses ranged from 125 mg to 500 mg twice daily, and all 5 were judged at high risk of bias. Based on very low certainty evidence, there was no difference between bacopa and placebo or donepezil. No major safety issues were reported.
Other outcomes. The 2024 review summarized 22 clinical trials reporting improvements in memory retention, attention, learning, mood and sleep, and lower inflammation markers, and called bacopa a possible complementary approach.
Why the evidence is still uncertain
- Small, biased trials. The 2022 review rated all 5 Alzheimer's trials at high risk of bias and downgraded the evidence for small samples and wide confidence intervals.
- Different measures. Three trials used the Mini-Mental State Examination, one used an Alzheimer's cognitive scale and two used test batteries, making comparisons hard.
- Mixed products. Three of the five trials combined bacopa with other herbal extracts.
- Short follow-up. The memory trial ran 12 weeks of bacopa followed by 4 weeks of placebo, 16 weeks in total.
- Broader reviews. The 2024 review describes reduced inflammation and oxidative stress markers and improvements in attention, impulsivity and hyperactivity across trials, but more rigorous studies are needed.
The 2022 authors note that standard Alzheimer's drugs can cause side effects such as nausea, vomiting, diarrhea and insomnia, which is one reason people look at herbs, but bacopa has not been shown to replace them.
Safety and interactions
- Stomach upset. NOW Foods notes bacopa may cause gastrointestinal discomfort in sensitive people. Trials reported nausea, constipation, drowsiness, rash and an aftertaste in some participants.
- Medicines. NOW Foods advises consulting a physician if you take MAO inhibitors, cholinergic or anticholinergic medicines, acetylcholinesterase inhibitors such as Alzheimer's drugs, or thyroid hormones.
- Thyroid. NOW Foods specifically flags hyperthyroidism.
- Pregnancy. NOW Foods advises a physician check if pregnant or nursing.
- Not a dementia treatment. See a doctor for memory loss.
Doses and standardization
- Studied doses. The memory trial used 125 mg twice daily; Alzheimer's trials ranged from 125 mg to 500 mg twice daily.
- Bacosides. NOW Foods' Bacopa Extract gives 450 mg standardized to at least 40% bacosides, taken once daily. Other products list different bacoside percentages or whole-plant extracts, so amounts are not directly comparable.
- Give it time. The memory benefit was measured over 12 weeks.
How to choose a bacopa supplement
- Standardized extract. Look for a stated bacoside percentage, as in the studied extracts.
- Single herb. Many brain blends add other herbs; a single-ingredient product matches the main memory trial.
- Watch for stomach upset. NOW Foods notes sensitive people may have digestive discomfort.
- Review your medicines with a pharmacist, especially thyroid and Alzheimer's drugs.
- Set expectations. Benefits in trials were on specific memory tests, not dramatic changes.
